\nThere is not a specific place to indicate the mode of response, but it would probably go in the task description; it also would be apparent in many of the evidence types. For example, if the mode of response is eye gaze, and you use captioned photos as one type of evidence, you will document that the student made a selection by looking at the item presented, and the pictures will show the student doing that. I'll let Jim comment on any implications this might have for the CoED. Congratulations on getting that binder sent in.

\nYou've raised a good question that Kathleen and I discussed many times over the summer. We finally decided to keep the data and checklist forms as they are, with electronic entry in the top portion and manual entry in the data collection area. We based this decision primarily on our consideration of the amount of time and effort that would be required to add and track many additional text fields covering three different data charts and a checklist, each requiring four unique entry sheets. MORE TIME AND EFFORT THAN WE COULD MUSTER! We also believed that, even if we did this, most teachers would probably still be manually writing the data collection on paper at the scene of the task, and later transcribing their notes into a computer (a "two step" versus "one step" process.) How many teachers have access to a laptop, or even an Alphasmart? When we considered questions like this, we convinced ourselves to "not pursue the electronic data\/checklist forms", at least not now.
\nDave, you may be aware of another option that is available to you. On the ODE website, you can download Microsoft Word documents with imbedded text boxes for all of the fields on each of these same cover sheets. Of course, with these, you lose the benefits of having a database to "autofill" most of the other fields on the form. The choice is yours. I know this probably not what you here hoping for.
